Perimenopause care in the 60 largest US cities (2026 data)
How many OB-GYNs, women's health nurse practitioners, nurse-midwives, sleep doctors and endocrinologists practice in America's largest cities? We counted them in the federal NPI registry.

Key findings
- Front-line menopause care is mostly nurse-led in numbers. Across the 60 cities we found 4,027 women's health nurse practitioners and 3,471 nurse-midwives, compared with 3,332 OB-GYNs.
- Sleep specialists are scarce. Only 1,315 sleep medicine doctors were listed, although sleep problems are among the most common complaints of perimenopause.
- Counts are concentrated. The five cities with the most clinicians (New York, Boston, Chicago, Houston, Dallas) account for 24% of everyone we counted. Large academic medical centers weigh heavily in these totals.
- Most clinicians are women. 77% of the clinicians counted are women, which matters to many patients looking for a clinician they feel comfortable with.
These are raw counts, not rates per resident: a larger city will usually have more clinicians. They show where care is listed, not how easy it is to get an appointment.

Method
- Source: the CMS National Plan and Provider Enumeration System (NPPES) NPI Registry public API, individual providers only (NPI type 1), queried in October 2026.
- Who counts: clinicians whose registry taxonomy matches exactly one of five specialties (OB-GYN and its subspecialties relevant to midlife care, women's health and OB-GYN nurse practitioners, advanced practice midwives, sleep medicine, and endocrinology, diabetes and metabolism), with a practice address in the named city.
- Cities: the 60 largest US cities by population. Suburbs and neighboring cities are not included, so metro areas are undercounted.
- Limits: the registry records what clinicians declare. It does not show whether a clinician sees perimenopause patients, accepts new patients or takes a given insurance, and some listed clinicians may no longer practice.
